Male, 37 years old

Reason for consultation

Abdominal pain radiating to external genitals

Clinical history

Yesterday morning, on the way to work, he suddenly felt violent pain in the abdomen. The pain presents with paroxysmal colicky type on the left side of the umbilicus that radiates to the hypochondrium of the same side and to the genitals. The pain rejects palpation. Additionally when it hurts, he has urge to urinate and vomits clear fluids. He has general cooling of the body. The face is pale. Normal tongue with white and moist coating. Deep and tense pulse

Diagnosis

  • Cold stagnates the Liver channel
    • Sensation of distension in the lower abdomen with pain descending toward the scrotum and testicles and rising toward the hypochondrium (pain is relieved with heat); cold sensation, cold hands and feet, vomiting of clear and watery fluids or retching, pale and moist tongue with white coating, tense and slow pulse.

Cold: pale face, pale tongue, sensation of cold.

Liver channel: the sudden pain along this channel.

Treatment

  • Cold stagnates the Liver channel:
    • CV-4 (with moxa): (crossing of LR, also CV-2 and CV-3);
    • ST-36 (all abdomen problems, and increases vital energy);
    • LR-3, LR-8; heated needle
    • Acute pain of LR channel: LR-1, LR-3, LR-5 (palpate to choose)
